These terms govern your use of GOAL.MD's website and telehealth services. Please read them before using the service or requesting treatment.

Telehealth services

GOAL.MD connects patients with licensed U.S. physicians who evaluate whether treatment is appropriate. Using this site does not create a physician-patient relationship until a licensed physician accepts you as a patient. No treatment is guaranteed, and a physician may decline to prescribe.

GOAL.MD serves patients in all 50 US states. Which medication your physician can prescribe still depends on your state's rules, your medical evaluation, and pharmacy eligibility.

Payment, renewals and cancellation

Brand-Name GLP-1 Access Program — Membership & Billing Terms

In plain English: Brand-Name Access is a physician-care membership — $95 per month, or a lower per-month price if you choose a 3, 6, or 12 month cadence. It renews automatically while you are enrolled so your physician supervision, prescriptions, and unlimited Nutrition Coach support never lapse. You can cancel any time before your next monthly prescription or refill is authorized. Once that refill has been authorized, the current cycle proceeds as scheduled and your cancellation applies to the next cycle. Once a payment has been processed and your physician has begun clinical work for that cycle, that payment is final and non-refundable, because the work has been performed. The fee covers physician services only — your medication is paid separately to the manufacturer. This is different from GOAL.MD's compounded medication programs, which are never auto-billed: you authorize each compounded refill yourself. Questions: customer@jellyfish.md.

1. Program Description

The GOAL.MD Brand-Name GLP-1 Access Program (the "Program") provides physician supervision for patients whose physician determines that an FDA-approved brand-name GLP-1 medication (such as Wegovy®, Ozempic®, or Zepbound®) is the most clinically appropriate treatment. The Program covers physician evaluation, prescription management, dosage adjustments, ongoing clinical supervision, unlimited Nutrition Coach support, and care-team messaging. The Program does not include the cost of the medication itself, which the patient purchases directly from the manufacturer's program (e.g., LillyDirect™, NovoCare®) or a participating pharmacy.

2. Program Fee and Billing Cycle

The Program fee is $95.00 USD per month of physician supervision. Patients may instead select a longer billing cadence at a lower per-month price: $255.00 billed every three months, $450.00 billed every six months, or $780.00 billed every twelve months. The fee is for professional physician services and is fully earned upon completion of the physician's clinical work for the cycle.

An authorized introductory discount of $50.00 may be applied to a patient's first Program payment only. The discount is decided by GOAL.MD's payment service, applies once, and does not change the amount of any later cycle.

3. Automatic Monthly Billing While Enrolled

Enrolling in the Program authorizes GOAL.MD to bill the patient's payment method automatically at the start of each billing cycle for as long as the patient remains enrolled. This is deliberate: continuous billing is what keeps physician supervision, prescription authority, and unlimited Nutrition Coach support active without interruption in care.

The patient's payment method is stored securely with GOAL.MD's payment processor for this purpose. The amount billed each cycle is the standard price for the cadence the patient selected.

4. This Is Different From Compounded Medication Orders

GOAL.MD's compounded medication programs are never automatically billed or automatically renewed. For those programs the patient must explicitly authorize each refill, and doing nothing simply means nothing is charged and nothing ships.

Automatic billing described in these terms applies only to the Brand-Name GLP-1 Access Program, which is a professional-services membership and does not include medication.

5. Cancellation

You can cancel any time before your next monthly prescription or refill is authorized. Once that refill has been authorized, the current cycle proceeds as scheduled and your cancellation applies to the next cycle. GOAL.MD does not require any advance-notice window before cancelling.

To cancel, contact GOAL.MD at customer@jellyfish.md or use the patient portal. Cancellation stops future cycles; it does not refund a cycle already paid for in which clinical work has begun. See Section 9.

When the Program ends, physician supervision under the Program ends and active prescriptions are not renewed after that point.

6. What the Fee Covers (and Does Not Cover)

The Program fee covers physician evaluation, e-prescribing to the manufacturer's pharmacy network, dosage titration and adjustments, clinical follow-up, care-team messaging, and unlimited Nutrition Coach support. The fee does not cover the cost of the brand-name medication (paid directly to the manufacturer program or pharmacy), laboratory work, imaging, or any services obtained outside GOAL.MD. GOAL.MD is a cash-pay practice and does not bill insurance, Medicare, Medicaid, or any third-party payer.

7. Price Changes

GOAL.MD reserves the right to adjust Program pricing. GOAL.MD will notify enrolled patients by email before a price change takes effect on their membership, and the patient may cancel under Section 5 if they do not wish to continue at the new price.

8. Declined Payments

If a cycle payment is declined, GOAL.MD's payment processor may retry the charge and GOAL.MD will contact the patient. Physician supervision, prescription authorization, and Nutrition Coach support may be paused until a payment succeeds. Continued non-payment ends enrollment.

9. Once Paid and Clinical Work Has Begun — Payments Are Final

A Program payment becomes final, non-cancellable, and non-refundable once both of the following have occurred:

(a) the payment has been successfully processed, AND (b) the supervising physician has begun clinical work for that cycle — including, without limitation, chart review, dose evaluation, refill approval, or e-prescription transmission.

This rule applies regardless of whether the patient subsequently:

10. No Partial Refunds / No Proration

Because the Program fee covers a defined period of physician availability and clinical work, payments are non-refundable and are not prorated for unused days, missed appointments, declined prescriptions, or a decision to stop partway through a paid cycle.

11. Returning After Cancellation

A patient who has cancelled may re-enroll at any time. Depending on how much time has passed and the patient's clinical circumstances, a new physician evaluation may be required before prescriptions resume. The introductory discount in Section 2 applies to a first Program payment only and is not available again on re-enrollment.

12. Medication Availability & Manufacturer Programs

GOAL.MD does not control medication pricing, supply, or shipping by Eli Lilly, Novo Nordisk, or their affiliated pharmacies. Manufacturer-side changes (price increases, eligibility changes, shortages, or program termination) do not entitle the patient to a refund of the GOAL.MD service fee for any cycle in which the physician has already reviewed and approved the refill. If the physician determines that brand-name therapy is no longer appropriate or available, the patient may transition to another GOAL.MD program.

13. Distinction From Compounded Medication Orders

This Program covers physician services only and is governed by these membership terms. It is separate and distinct from the "Order Finalization, Cancellation & Refund Policy" above, which governs custom-compounded medication orders. Compounded medication orders are never auto-billed and remain non-cancellable and non-refundable once finalized, as set forth in that section.

14. Disputes & Chargebacks

Initiating a chargeback for a Program charge — particularly one where the physician has already performed clinical work for the cycle — without first contacting customer@jellyfish.md to attempt resolution may result in Program termination and will be disputed by GOAL.MD with documentation of the patient's enrollment authorization and the physician's clinical work performed.

15. Patient Acknowledgment

By enrolling in the Brand-Name GLP-1 Access Program, the patient acknowledges they have read, understood, and agree to:

You must be at least 18 years old and provide accurate medical information. Clinical eligibility, state rules and pharmacy requirements determine whether a specific medication can be prescribed and dispensed to you.
